| This service provides two levels
of care in a community based program which renders day supervision.
This includes a variety of health, therapeutic and social services,
as well planned activites offered in a group setting. Services
include nursing care, special therapeutic services, personal care
services, planned therapeutic activities, dietary services, transportation
and social work services.

Emergency Reponse
Services |

Emergency
alert response service is a community based electronic surveillance
service which monitors the frail home bound elderly by means of
an electronic communication link with a response center.

Home Delivered Meals
|
|
This service insures
improved nutritional support which helps to enhance the consumers
health and well being. Consumers receive excellent meals at home.
This service must be offered only in conjunction with other CCSP
services.

Home Health Services
|
|
Ordered
by a physician, includes nursing services, physical, speech,
and occupational therapies, home health aides, and medical
social service to individuals in their homes. |

| ICWP (Independent Care Waiver Program) |
|
The Independent Care
Wavier Program started in 1992 offers services that help a limited
number of adult Medicaid recipients with severe physical disabilities
live in their own homes or in the community instead of institutional
settings. The services offered through ICWP are a supplement to
the care that can be provided to individuals by their family and
friends in the community. |

Personal Care
Homes/Alternative Living Services |
|
Under
the Community Care Services Program, care in a personal
care home (also known as assisted living facilities) is
known as Alternative Living Services.
An ALS client in CCSP receives 24-hour supervision and care
in a licensed family or group model Personal Care Home. Nurses
develop a plan of care the PCH staff is to follow and evaluate
at least twice monthly the care ALS clients are receiving.
The nurse also reviews MD medication orders monthly and assures
that the ALS client is taking medications as ordered and that
the PCH's medication records are accurate |

Personal Support
Services |
|
The
CCSP provides Personal Support Services to individuals at risk
of placement in a nursing facility. The intent of the services
is to proved quality, competent care to CCSP clients in the form
of personal care, housekeeping, home management, proper nutrition,
medical-related activities, ambulation, and respite care to caregivers.
These services are expected to maintain or increase the functioning
capacity of clients being served and focus on the relationship
between the client and the client's needs (1-1 1/2 hours maximum
visit time).

| PHCP (Private Home
Care Provider) |
|
A PHCP is a company
that is licensed and surveyed yearly by the state to provide
up to three types of services.
Services that are regulated may include one or any mix of
the following provided in a private home: 1. nursing services,
2. personal care tasks, and/or 3. companion or sitter tasks
(O.C.G.A. 290-5-54-.03).
The employee(s) may coordinate and provide skilled nursing
care (nurse) as well as tasks such prepare and feed meals or
simply be a sitter (unlicensed companion/sitter). |

PSSX Extended
Personal Support Services |
|
The
CCSP provides Extended Personal Support Services to individuals
at risk of placement in a nursing facility. The intent of the services
is to proved quality, competent care to CCSP clients in the form
of personal care, housekeeping, home management, proper nutrition,
medical-related activities, ambulation, and respite care to caregivers.
These services are expected to maintain or increase the functioning
capacity of clients being served and focus on the relationship
between the client and the client's needs This service is also
for the purpose of relieving the person(s) normally providing care
and/or oversight (3 hours minimum maximum visit time).

to improve the health outcomes of persons with chronic conditions,
through the development of a cost-effective, comprehensive managed
care model. SOURCE is distinguished from other wavier programs
in Georgia by the linkage of primary medical care to community
services, through a Primary Care Physician/ Case Manager team. |
